Sports Physicals and Medical Information Forms

All students participating in Odell Grade School Sports during the 2009-2010 school year must have a current sports physical, medical information, and insurance waiver on file at the OGS office to participate in all practices and sports teams.  Physical forms are good for one year, therefore, if your child's previous year's physical forms are current at the beginning of practices, they must be replaced with next year's on the date the current physical expires to continue to practice.  If you are not sure of the date of your previous year's physical, contact the OGS office for that information, 815-998-2272.
